Consent to the processing of personal data

CONSENT TO THE PROCESSING OF PERSONAL DATA

I hereby, in accordance with Federal Law No. 152-FZ On Personal Data dated July 27, 2006, freely, of my own will and in my own interests, express my unconditional consent to the processing of my personal data by Alfarma LLC, registered in accordance with the legislation of the Russian Federation at: 127238, Moscow, 3 Nizhnelikhoborsky proezd, bldg 1A, floor 4, prem. X, room 12 (hereinafter referred to as the Operator).

Personal data means any information relating to a specific individual or an individual determined on the basis of such information. I have given this Consent to the processing of the following personal data: Name, phone number, e-mail, IP address.

The Consent is given to the Operator to perform the following actions with my personal data with/without using automation means: collection, systematization, accumulation, storage, clarification (update, change), use, depersonalization, transfer to third parties for the purposes indicated below, as well as the implementation of any other actions provided for by the current legislation of the Russian Federation, both in manual and automated forms. This consent is given to the Operator for the processing of my personal data for the following purposes:

providing the User with the Site services;
sending notifications regarding the Site services;
preparing and sending responses to the User’s requests.

The list of actions with personal data to which the User expresses his/her consent: collection, systematization, accumulation, storage, clarification (update, change), use, depersonalization, transfer to third parties for the purposes indicated above, as well as the implementation of any other actions provided for by the current legislation of the Russian Federation, both in manual and automated forms.

The Company undertakes to take all necessary measures to protect the User’s personal data from unauthorized access or disclosure.

This Consent is valid until withdrawal by sending a notification to the email: info@al-farma.com. In case of withdrawal of my consent to the processing of personal data, the Operator has the right to continue processing personal data without my consent if there are grounds specified in clauses 2-11 of part 1 of article 6, part 2 of article 10 and part 2 of article 11 of Federal Law No. 152-FZ On Personal Data dated July 27, 2006.
